Ready & Forward Aftercare Program

Outpatient clients can transition to our Ready & Forward Aftercare Program to pave the way for reintegration into the community while maintaining independence and sobriety. This program addresses a veteran’s specific life challenges by pairing the continuation of individual and group therapies designed to prevent relapse and enhance sober living skills with case management services that ensure our veterans access the available resources and tools to achieve long-term success.
